How to sign up: It’s easy! Please email your name, mailing address, phone number, and email address to info@cwsnaturally.com with Referral Program in the subject line. We will get back to you in 1-3 business days.
What happens next: A unique coupon code will be assigned to you and emailed to the address provided. The code will be active when it is emailed to you. The code is valid until December 31, 2010. Get your family and friends to order at our online store and use the coupon code during check out. That’s it.
How you receive payment: Monthly, you will receive a statement of orders for your unique coupon code and a check for 10% of the product sales amount (excluding, shipping and taxes) from each order.
For product sales totaling $5,000 from January 2010 - December 31, 2010, you will receive a bonus check for $250. That’s like an overall commission of 15%!
For product sales totaling $10,000 from January 2010 - December 31, 2010, you will receive a bonus check for $500. That’s like an overall commission of 17.5%
To receive payment of your earned commissions, the IRS requires that you fill out and provide us with your IRS Form W-9, which you can download at the IRS website at http://www.irs.gov/pub/irs-pdf/fw9.pdf?portlet=3. You must sign and send the completed form to us either by fax at (763) 551-2572, or by U.S. Mail at 1864 Berkshire Lane North, Plymouth, MN 55441-3723.
